Please be aware of the co-pays and/or deductibles that are due at the time of your appointment. If you are not sure, call us before your appointment and we can help you figure it out. Our contract with your insurance company obligates our office to collect co-pays. Moreover, collecting co-pays and deductibles is essential for our ability to keep our doors open for service.
For your convenience, we accept cash as well as credit and debit cards. We do not accept personal checks.
Our office staff can answer any question that you may have about our financial policy.

example, you may receive up to 3 SMS messages per week related to your
future appointments.

Please note that standard message and data rates may apply, depending
on your carrier’s pricing plan. These fees may vary if the message is
sent domestically or internationally.
– Opt-In Method:
You may opt-in to receive SMS messages from Smart Heart & Medical Care
