Please be aware of the co-pays and/or deductibles that are due at the time of your appointment. If you are not sure about the amount, please contact us before your appointment, and we will be happy to assist you.

Our contract with your insurance company requires us to collect co-pays. Collecting co-pays and deductibles is essential for our ability to continue providing quality care and keeping our doors open for service.
